The project “Versorgungsszenario Geriatrie-Akte” (“Usage of a Geriatrics Care Record“) uses electronic case records to improve safety for patients and their relatives. This requires transparent information and communication. Transitions in healthcare must be better designed to avoid mismanagement. Although the ECR is a physician-led, cross-enterprise patient record, it is up to the patients to decide which organizations have access to their data. The ECR will enable various participants in the care process, from individual doctors to hospitals and rehabilitation clinics, to access the patient’s relevant medical information.
FAC’T Group, which belongs to the St. Francis Foundation in Münster, one of the largest denominational hospital owners in Germany, plans on using the platform in its own facilities.
